Mega Casanova 3 is the third entry in the Mega Casanova series and, like its predecessors, is based on a Java mobile game later ported to the Sega Mega Drive. The story begins after the protagonist is involved in an accident and wakes up in a hospital, where he becomes acquainted with three nurses. Over the course of the game, the player must decide which of them to pursue.
Gameplay remains largely unchanged from the previous titles, focusing on dialogue choices and branching story paths. This installment introduces a handful of simple logic-based puzzles that add a small amount of interaction beyond conversation. The game also features three equally important endings, encouraging players to explore different choices, though the overall experience remains brief and mechanically limited.
